Kevin is Director and works with Nikki and the team. Kevin focuses on providing community training, working with schools to develop well- being programmes and provides longer term support for people experiencing issues with their mental health.
Sharon and Libby are Social Prescribers working in the community and setting up support groups or working with local community organisations.
Nikki leads the community development work and setting up new projects working with a wide range of organisations across Cornwall and works with Sharon and Libby.
Anna leads on our website and digital toolkit and supports the team with programme development.
Imagine If is supporting the development of an innovative approach to social prescribing and community development in Cornwall - Community Connect Cornwall. The aim is to connect people to their communities to reduce isolation, improve well-being and build thriving resilient communities.

Community Connect, Cornwall is currently developing
I Belong community model of social prescribing is to encourage and improve well-being, increase and widen community response and skills to cope and share support and reduce pressure on GP surgeries and A&E services.
Community Connect offers
o one to one support for those referred from GP surgeries (linked to NHS 10 year Plan)
o responses managing the impact of COVID 19 on individuals and communities
o partnerships with local groups offering support around loss and grief
o Dementia-friendly approaches for towns and communities
I Belong social and emotional learning programmes for schools focused on skills to support well-being, learning and future employment
o currently working with Chacewater and Devoran schools
